Do I have ADHD, ADD, or is it just my hormones..?

1. SOMETIMES when I'm reading, I don't get engaged in it as much as I used to
2. When teachers ask questions in class, I really wanna speak up but I eventually fail to because I run over answers in my head like 40 times
3. My dad says I've been wayy too shy lately and forgetful and selfish
4. I get yelled at a lot for forgetting to turn off lights, fans etc


please help..I'm in sixth grade. I don't want to have ADHD or anything because I have major goals in life to be successful and make it into good schools.
The only thing im good at it seems is piano.

Oh yea some people say that im too "mechanical" and I can only solve problems written out but not like logical word problems. I panic wayy too much
