When a fracture patient comes to the first aid clinic writhing in pain, is he given a pain shot immediately?



I am trying to understand a medical procedure.

My friend recently broke his lower leg after falling from a train and was immediately taken to a rural small clinic nearby.

When I reached there after about 1/2 hour of the incident he was not writing in pain, but I was told that when he was brought there he was shrieking. His outer wounds were minimal and were dressed. But he would shriek if anyone touched his leg.

The nurse gave him a butt shot of pain killer 15 minutes after I reached there.

I now wonder if the nurse had given one shot of pain killer earlier (as soon as he arrived), because otherwise how come my friend was not writhing in pain. But his pants were still on and the belt was tight when I reached there (which had to be opened to give the shot), so it seems he had not been given that shot earlier.

What is the normal practise in India for giving first aid to frac

No, not immediately. They have to be seen by a physician first and an order will be given to the nurse to give the pain killer.

Nurses must protect their own licenses and giving a pain shot without an order is against their license.

Also, doctors know more about pharmacology and they have to take in effect medical issues and pain killers such as allergies and what not.

If this was in the public and there were first responders (paramedics), then they can practice within their own scope to give a painkiller (morphine) intravenously.

Doctor's have to examine the patient first and have the patient answer questions about the pain before any pain relieving medication can be administered.
Definitely not str8 away. one needs to see what is wrong to ascertain what level of pain relief will be required, to find out if the patient is allergic to anything, if the patient needs to be operated on they cant have a combination of pain relief plus anesthetic
I don't know about in India, but here in the US they'll let you sit there and scream until a doctor is free to come and evaluate you. They always say the doctor needs to come and see how much pain you're in, which will help him decide what's wrong with you. I dislocated my shoulder last year, badly. I would look down and to my right and be looking at the back of my shoulder. It's the worst pain I've ever been in, but they let me sit there screaming until a doctor was free. And the nurse doing admissions kept trying to hand me a pen and a clipboard telling me to fill out my paperwork. And occasionally someone would pop in to tell me to "shut up, you're bothering the other patients" It sounds to me like your friend got off easy ...
Medical professionals need time to asses an injury and must decide on pain relief based on many factors including what the injury is, how it happened, whether the person took drugs or alcohol before the incident , x-rays and whether pain relief might interfere with anesthesia if surgery is necessary. A huge portion of the shrieking on entering the hospital was most likely a panic reaction. Of course he was scared. The trauma of the incident, the pain itself and the endless string of possible outcomes going through his head. It would make anyone panic. I doubt that he was poorly treated but it is always frustrating when you see someone you care about in pain and can't do anything to help.
