Why red blood cells are irradiated before transfusion and why stem cells and plasma are not irradiate.?

Many units of red blood cells and platelets ARE irradiated before transfusions. This is done to inactivate lymphocytes in the donor units. Immunocompromised patients with various leukaemias and lymphomas who may be candidates for marrow transplants are put at risk of GVHD (graft versus host disease if they receive blood that has not been gamma irradiated. The lymphocytes from the donor can actually graft and start rejecting the host (ie. the recipient of the blood). Plasma is not irradiated because it has had all the cells removed from it.

Cells are not irradiated prior to transfusion as they will not function after radiation. Plasma does not contain any cells. Packed red cells are used for most general blood transfusions. White cells and stem cells need to tissue cross matched before transfusion to prevent conflicts with the host immune system.

Most blood in the US is not irradiated prior to transfusion. Irradiated blood is generally only given to a patient when requested by their doctor. Liz is correct in stating when (clinical conditions) doctors request irradiated units.
