No, it doesn't hurt. Yes it can spread and kill you if left undetected. That's why you should get a pap every year to catch it. They found mine that way. I had what was called a loop. they removed a piece of my cervix, it was done in the DR office. They caught it quick since I had an annual pap. The piece of cervix they removed did not stop me from having children. It was crampy for awhile, felt like a bad period.
