Does depression affect your decision making?

yes, most definitely. depression can cause a lot of weird thoughts and effect almost everything you do and think

Oh, and when the depression is really, really bad, you don't think about your past mistakes made while depressed. You're just in utter hopelessness. There is no rationalizing in that situation.

If untreated, yes. But I'm talking about Clinical Depression. Not the "I'm so depressed" that people always claim and take lightly.

deciding not to do somthing you should ,or have to ,because of depression i would say is an affect .so yes.
