Health Insurance sucks - how do I prepare for another sickness?

I used to go to this GP for a physical, and my insurance is supposed to cover the full cost for an annual physical but each time the insurace company made me pay an extra 100 or so out of pocket because the physical costs 'more than estimated.' This GP was even within the network of doctors I was instructed to go to by the insurance company!

Anyways is it weird to ask upfront for the cost of a physical or for any medical examination or procedure, and to get it in writing? And then call the insurance company to make sure that they cover this cost, and then get that in writing? It just seems like kind of a weird bureaucratic process to go through in order to budget for any sickness or routine physical, but is this what other people in the United States do?
