Where can I get a form for a hepatitis A vaccine?


My doctor will not give me a hepatitis A vaccine, even though my insurace covers it, unless I have a form from church/school/travel saying I actually need it.

well why do you need it? The person who told you should give you a form
if you work with alot of people,i think you should get it.go to your health dept. and ask, it might be free.
