Does anyone know the first line of treatment for infertility problems?

First I would go see your OB/GYN. There are many basic fertility tests they can perform before referral to a Fertility Specialist. I went to my OB/GYN complaining of irregular periods. Also mentioned to him that my husband and I have been TTC with no luck for about a year. He ordered routine labs, CD 3 hormone levels, and a trans vaginal ultrasound. Also requested a semen analysis from husband. Friday had one last test, progesterone level. If this is normal as well, we will start on Clomid if no results, he will then send me to an RE for further evaluations. By Starting with the OB/GYN and my original diagnosis of irregular periods, all tests have been covered 100% by insurance.
(my insurance does not cover anything to do with infertility)

First course of action would be to check you and your partner to make sure you're ovulating each month and his sperm count is sufficient. Then the doctor can diagnose the problem (if there is one) and choose the appropriate treatment. Treatment options can range from a simple prescription for Clomid, which costs ~$25/month and you take it for 5 days at the start of your cycle, to IVF/ICSI which can cost up to $20,000 per cycle, depending on the clinic.

I would recommend consulting a Reproductive Endocrinologist (RE) rather than an OB/GYN as they have far more experience with infertility.

I'm 25 and have been TTC for 8 months also and my doc agreed to see me. I actually am going this afternoon. He said he is going to run some bloodwork on me and go over cycle history and possibly test my hubby. Given my age and the fact that I have only been TTC for 8 months I don't think he is going to go deep into it just yet unless the bloodwork comes back abnormal. But the fact that you are 41 and TTC 8 months, they will definitely see you. Each doctor has different steps they will take given your specific situation. Good luck!

Since you're over 35, you should definitely see a fertility specialist right away. Don't try to go it alone, you could be wasting precious time!
