The doctor has to sign the form. does it hinder your mobility? No but I'll make you a sticker that says "feed me milk" how lazy are you? Get the request form and have your doctor sign it and then you turn it in. Most states require that you cannot walk more than 100 ft or so in order to qualify if you are still ambulatory. Check with your DMV, if you qualify,then get a prescription from your Dr. You can if your physician completes the necessary paper work stating that it is MEICALLY NECESSARY and that you have a walking disability. If you truly have a disability it should go through without any problems. It depends on if you are having problems getting around. Just because you have been diagnosed, doesn't mean you are disabled and need extra help when getting around. If your osteoporosis is interfering with your mobility, your doctor will be able to request a decal for you. |
