Schizophrenia or other Mental Disorder?


My friend has told me that she has been having problems, but wants to wait before seeing a psychiatrist. Her symptoms are:
1) she cuts her self A LOT (over 62 times a month)
2) says that Bob tells her to cut and ignores/pesters her if she doesn't
3) Bob is a voice in her head
4) can't sleep without having nightmares
5) "sees things" all the time (i.e. a lady in the corner of her room, a person behind her in the mirror, someone stepping OUT OF the mirror, dead bodies, etc...)
6) says that the people all want to kill her
7) says she can feel what's happening to her in her nightmares (most frequently being chopped alive) and feels sore in the morning
8) usually can't sleep
9) feels suicidal
10) feels like she's being watched ALL the time.

She has other symptoms I think that she's not telling me.

Don't answer this to say: She's a liar. Or something stupid like that.

She's told me this stuff over WEEKS of talking.

What is the most likely Mental Disorder (if any) that she might have?

I think this is severe psychotic depression. I think this because all her hallucinations (the voice) and delusions (false beliefs "people want to kill her") are mood congruent- they match periods of low mood, cutting and suicidal thoughts. The other clue is that she says Bob is within her own head, rather than an external voice. Visual hallucinations in schizophrenia are quite uncommon whereas they are much more common in psychotic depression and always of a morbid theme.

I'm very very concerned for your friend. You need to earn her trust enough to get her to call you whenever she's feeling suicidal- it's a big weight on you but you need to be the one to call the police or ambulance if you think she's really going to hurt herself. You also need to encourage her to see a doctor straight away. I appreciate your friend is probably scared about being called 'crazy' and having to face her problems but this is a serious illness and she needs help.

also if u feel that she mite be a danger to herself and to others i really think that u should encourage her to get help today, or u should talk to someone to get her help immediately.

I think the best answer is, she needs help. don't worry about what it is-let a doctor figure that out! Please tell someone, her parents or someone trustworhty, that she needs help!

Paranoid Schizophrenic would be my first assumption and I come from a long bloodline of several rapid cycle bi-polar sufferers and have had my fair share of time in psychological treatment. Get them some help ASAP!!

All of these symptoms do sound like a psychotic type disorder such as Schizophrenia or Depression with psychotc features or another type of illness.

Whatever the case, she is in serious need of help.

That's good that she trusts you but you really need to urge her to get help, talk to her parents if she is underage, talk to your parents if you are underage and if neither of you are underage, contact county mental health services and they can help you help her get the help that she needs.

It really sounds like she needs to be in the hospital so that she can get stabalized real fast.

Wow, This does sound strange, and if you're sure she is serious it sounds like she is having delusions and/or hallucinations. I think you need to convince her to go to the doctor and explain what's happening. It could be a number of mental disorders, (like schizophrenia) but they usually get worse over time so it's better to get her checked out now - they might be able to give her some medication/therapy that can put her back on the right track. If not for these thoughts about 'Bob' the voice in her head, she should at least get help for her self-harm. Please seek help for her, as nobody on here can diagnose her, she needs a doctor to see the full picture. You sound like a good friend and she must trust you to tell you about this.

If you can't convince her to go to the doctor, maybe you could try speaking to her parents. She might be mad at you for doing it, but it sounds like she really needs help. If this is not an option, you could try speaking to the doctor yourself about her, but this is difficult as some doctors will only talk to the person themselves that's having the problem. It's worth a try though. Did she tell you why she wants to wait? I really don't think it's a good idea...If she's scared maybe you could try and reassure her that all the psychiatrist wants to do is help... I think she needs to get some help as soon as possible.

EDIT: It sounds like schizophrenia to me, because of the seeing people, 'bob' telling her to cut, and the paranoid thoughts. However, it could also be a bad depression. Some people with very severe depression experience delusions and hallucionations like thinking they are going to be killed, or bodies being chopped up alive. But please get her to a psychiatrist.
