nurses do not diagnose clients with schizophrenia, they already have been diagnosed. they document and report symptoms of schizophrenia, which can include, paranoia, delusions, feelings of persecution, auditory and visual hallucinations, thought disorders, they also document and record side effects of medication and their emotional and physical needs.

Ineffective coping r/t underlying mental health e/b poor oral intake, or refusing medications, or locking self in room etc.
