Why does smoking increase the risk of stroke while taking birth control pills?

the warnings that come with birth control pills indicate that smoking increases the risk of cardiovascular side-effects. i would like to know why this is and if this risk is present if the consumer is simply in the vicinity of the smoker and is only receiving a small amount of second-hand smoke. Also does this risk only apply to cigarette smokers, or does smoking something lighter such as hookah still increase this risk considering hookah only contains .5% nicotene?

smoke causes the blood vessles to constrict and that makes it much easier to have a stroke. it applies to all smoking im sure.
