How much time after a stroke should a person figure that swallowing is the best it is gooding to get?

Lots of factors influence recovery from dysphagia due to stroke.
The type of stroke (ischemic or hemorrhagic), the location and size of the lesion, the presence of co-occuring disease (such as Parkinson disease), the presence of a tracheostomy, the patient's ability to understand and remember instructions (including alertness), and the presence, absence, and degree of weakness of the cough reflex (ability to protect the airway during swallowing).
Typically, the most dramatic recovery is within the first 3 months post-stroke. Further recovery is possible even later than that; the curve just isn't terribly steep.
I always recommended placement of a PEG or G-tube if I determined it would take longer than 2 weeks to regain enough function for adequate intake of foods and liquids. This provides a safe route for the administration of sustenance while lowering aspiration risk and is far more comfortable than having a nasogastric (NG) tube.
If you give me more information specific to the patient I may be able to give you a better idea of a possible prognosis.
